AMCAS vs. AMCAS: The Most Important Thing Ohio Pre-Meds Need to Know

Ohio MD programs all use AMCAS (American Medical College Application Service). Ohio DO programs use AACOMAS (American Association of Colleges of Osteopathic Medicine Application Service). These are two completely separate applications with different essays, different deadlines, and different strategies. If you are applying to both MD and DO programs, you must complete both applications independently.

AMCAS opens May 1 each year. The most competitive applicants submit by June 1 — the earlier you submit, the better your chances. AMCAS uses a different essay format than AMCAS, with separate prompts for each school. You will need to write school-specific essays in addition to your personal statement.

Ohio residency is a significant advantage for Ohio State University College of Medicine and Northeast Ohio Medical University. Both programs prioritize Ohio residents. If you are not an Ohio resident, your chances at these public schools are more limited — but private programs and DO programs are more open to out-of-state applicants.

The Ohio In-State Advantage — and Its Limits

Ohio residents applying to Ohio State University College of Medicine and Case Western Reserve University School of Medicine have a meaningful in-state advantage. Ohio State in particular prioritizes Ohio residents as part of its workforce development mission. If you are an Ohio resident with a competitive application, you should absolutely prioritize Ohio schools.

But in-state status is not a free pass. Case Western Reserve University School of Medicine's average MCAT of 518 and GPA of 3.8 are among the highest in the country. Being an Ohio resident gets you in the pool at Ohio State and NEOMED — it does not guarantee admission. Your application still needs to be exceptional.

For out-of-state applicants, Case Western Reserve University School of Medicine and the Ohio DO programs are more accessible than Ohio State or NEOMED — they use AMCAS/AACOMAS and have somewhat more flexibility on state residency. Both are competitive nationally, but they evaluate all applicants on the same criteria regardless of state residency.

I tell Ohio pre-meds the same thing every cycle: apply to both AMCAS and AACOMAS schools. Use your Ohio residency advantage for the public schools, but don't limit yourself to Ohio. The strongest applications I've seen from Ohio students apply to 15–20 schools total — 8–10 Ohio schools through AMCAS and 8–10 national schools through AACOMAS. That is the right strategy.
