New York City has more clinical opportunities than anywhere else in the country. Here is how to build the patient care experience that NYC PA programs want to see.
For the national overview, see our complete guide to patient care experience for PA school: what counts and how many hours programs require.
Direct PCE means hands-on patient contact where you are providing care — not just observing. PA programs distinguish between direct PCE (EMT, CNA, phlebotomy) and healthcare experience (volunteering, shadowing). Most NYC programs require 500–1,000+ hours of direct PCE.
Dr. Tomei's Perspective
Counts as direct PCE
FDNY EMS is the busiest EMS system in the country. EMT certification through FDNY or a private NYC EMS company provides hands-on patient care that PA programs universally accept and value. The clinical exposure in NYC is unmatched — you will see more acute pathology in one shift than most volunteers see in a year.
Counts as direct PCE
CNA work in NYC hospitals and nursing facilities provides consistent, hands-on patient care. NYC has a large network of hospitals and long-term care facilities that hire CNAs. The work is demanding but provides excellent clinical exposure and strong letters of recommendation.
Varies by program
Medical scribing at NYC hospital EDs and urgent care centers provides exceptional clinical exposure. You are in the room with the physician, hearing real clinical reasoning in real time. NYC has multiple scribing companies operating at major hospital systems.
Counts as direct PCE
Phlebotomy provides consistent patient contact and is widely available in NYC hospitals and outpatient labs. The work is straightforward but provides genuine hands-on patient interaction that counts toward PCE requirements.
May or may not count as direct PCE — verify with each program
Clinical volunteering at NYC hospitals provides patient exposure but may not count as direct PCE at all programs. Best used to supplement paid PCE roles rather than as a primary source of hours.
Student-run free clinics provide direct patient contact in an underserved setting — exactly what NYC PA programs value. These experiences also demonstrate commitment to health equity, which is a core value at CUNY and other NYC programs.
Student-run free clinic at Columbia University Medical Center. Pre-PA volunteers assist with patient intake, health education, and clinical support. Whether it counts as direct PCE depends on your duties and each program's definition.
Student-run clinic serving uninsured patients in Manhattan. Pre-PA volunteers work alongside PA students and attending physicians.
Mount Sinai student-run clinics in East Harlem and the Bronx. Strong focus on underserved populations — valuable for CUNY PA program applications.
Free clinics serving homeless and low-income New Yorkers. Direct patient contact in a community health setting.
